# Setup Your First Lead Form
1. Click **Lead Forms**.
2. Click **+** **Create New Form**.
3. Enter the details below:
* Campaign/Form Name
* Select **Category**
* Select **Account To Assign To**
* Click **Create Form**

Lead Form successfully created & you will be directed to the form setting.

4. Click **Delete** to delete the Lead Form.
5. Click **Yes, Delete Form** to confirm the deletion.
